Job Summary
We are seeking a dedicated and skilled Home Health Occupational Therapist to join our compassionate team. The ideal candidate will possess a strong background in patient care across diverse settings, including home health, outpatient clinics, and inpatient facilities. As an Occupational Therapist, you will evaluate patients’ needs, develop personalized care plans, and implement evidence-based interventions to promote independence and functional abilities. Your expertise will support individuals of all ages with disabilities, neurodevelopmental disorders such as autism spectrum disorders, and those recovering from acute or post-acute conditions.
Duties
- Conduct comprehensive assessments of patients’ physical, cognitive, and sensory functions to identify therapy needs.
- Develop individualized treatment plans focusing on assisting with activities of daily living (ADLs), mobility, and functional independence.
- Implement therapeutic interventions tailored to patients’ specific conditions, including neurodevelopmental disorders like autism spectrum disorder.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings.
- Maintain accurate medical documentation and manage electronic health records (EHR) efficiently in compliance with healthcare regulations.
- Provide education and training to patients, families, and caregivers on therapy techniques, adaptive equipment use, and safety measures.
- Support early intervention programs for pediatric patients and contribute to case management efforts for complex cases.
- Participate in quality improvement initiatives by applying evidence-based practices and maintaining current knowledge of physiology and anatomy relevant to occupational therapy.
Experience
- Proven experience working with individuals with disabilities across various environments such as home health, outpatient clinics, skilled nursing facilities, or inpatient settings.
- Demonstrated expertise in assisting with ADLs, discharge planning, case management, and medical documentation processes.
- Experience working with children with special education needs or students with disabilities within school-based programs is highly desirable.
- Background in acute care settings such as ICU or inpatient units provides valuable insight into complex patient needs.
- Familiarity with working with neurodevelopmental disorders including autism spectrum disorders supports effective intervention strategies.
- Knowledge of medical terminology, physiology, anatomy, and evidence-based practices enhances clinical effectiveness.
- Prior experience managing electronic health records (EHR) systems ensures accurate documentation and compliance with healthcare standards.
