All-4-One Home Healthcare Services, Inc. has an opening for an Full-Time LPN for Quality Assurance. Candidates must have home health experience to include knowledge of meeting visit frequency under MD orders as relates to approved Plan of care and monitoring Plan of Cares submission and be able to thrive in a busy and fast-paced environment.
Responsibilities will include working with the scheduling coordinator of disciplines ordered for client services and QA of clinical notes to ensure meeting care plan requirements for completion of visits and needed orders. Will assist as back-up for scheduling coordinator as familiar with frequency requirements. Will assist with reviewing the plan of treatment and the medical plan for patients receiving skilled nursing services for the patient admission visit and generate the initial 485 certification/485 recertification forms. Will scan all patient documents into medical home care system. Will QA 10% of patient charts based census monthly. Will track and monitor submission of certification plans and work with Intake department on referrals and back-up as needed.
Must be hard working, well organized and professional and able to multi-task, problem solve and the ability to prioritize tasks and step in where needed.
Job Requirements
· Current LPN license in the state of VA from an accredited school of nursing required
· Must have proper knowledge of Home Health regulations and Reimbursement
· Minimum of one-year Home Health experience required
