We are looking for a case manager or marketer that is wants to help grow our Fort Smith location and work up into a management or administrative position. If you have home care, case management and/or marketing experience and are ready to take charge of your career, this is the place. This position will work into an administrative position reporting directly to the owner and allowing the right person to be in control of the entire Fort Smith division of the agency.

Job Overview
The TCM is responsible for overall management of TCM clients within the organization and will work closely with clients and healthcare providers to identify and address the health-related needs of the client. The TCM will advocate for clients, coordinating services and identifying resources inside and outside of the agency, allowing them to live independently in their homes.
If you are an RN at a home care agency, we would love to split your responsibilities and let you take full charge of the nurse responsibilities with our elderly in home clients and do case management responsibilities as needed.
We are truly looking for someone that wants to be independent, work on their own and advance to an administrative position in the company.
Qualifications
- LPN, Social Services Degree or a minimum of 2 years verifiable TCM experience with the elderly in an in-home care agency
- Must meet all Medicaid and Department of Health qualifications for targeted case managers
- Strong communication and interpersonal skills
- Familiarity with community resources and healthcare systems
- Ability to work independently or as a team
- Excellent organizational and time-management skills
- Excellent documentation skills
- Prior case management work with an in-home care agency
Responsibilities
1. Assessments of new TCM clients
2. Creation and maintenance of a care plan relating to all of their dependency needs
3. Perform in-person monitoring visits with clients as required by Medicaid regulations to ensure their needs are being met. Verify the client has ample food, utilities are on, hosing accommodations are met, homecare services are being provided, etc.
4. Perform monthly monitoring visits either in the home or via phone as required by Medicaid regulations
5. Monitor client’s health and welfare
6. Monitor client’s living conditions for safety and help locate services to aid the client
7. Research and recommend supporting activities for client’s recreation and community involvement to facilitate an enriched standard of living
8. Utilize a team-based approach to manage client care and ensure all services are delivered in a timely and efficient manner
9. Maintain current client charts as required by governing State and Federal agencies
10. Compliance with all required State and Federal guidelines for client interaction, including but not limited to monthly and quarterly monitoring
11. Provide education to clients and families to promote patient self-management and improve health outcomes if possible
12. Maintain accurate documentation as required by Medicaid regulations
13. Determine what services are available to client to aid in their ability to age at home and plan and implement those services according to the client’s self-directed (as applicable) plan of care
14. Assess and assist TCM clients with billable referral linkage services
