Responsibilities
- Administer medications, including IV infusions and injections, following strict protocols and documentation standards
- Manage complex care plans involving pain management, symptom control, and comfort measures for terminally ill patients
- Coordinate with interdisciplinary teams to develop and implement personalized care strategies that align with patient wishes and long-term care regulations
- Provide education and support to patients and families on disease progression, medication administration, and advanced directives
- Maintain accurate medical records using Electronic Health Record (EHR) systems
- Respond promptly to calls and urgent situations, providing guidance on acute pain management or infectious disease concerns
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification
- Proven experience in hospice care or palliative nursing environments; experience with hospice-specific protocols is highly preferred
- Strong knowledge of medical terminology, anatomy, physiology, and pharmacology related to end-of-life care
- Experience working with individuals with disabilities or complex chronic conditions such as dementia or Alzheimer's disease
- Excellent communication skills for caregiver education, client communication, and interdisciplinary collaboration
- Ability to handle sensitive situations with compassion while maintaining clinical accuracy and professionalism
