Pride Health is hiring an RN Home Health to support our client’s medical facility based in Milwaukee, WI 53226. This is a 13-week assignment with a contract-to-hire opportunity and a great way to start working with a top-tier healthcare organization!
Job Title: RN Home Health
Location: Milwaukee, WI 53226
Weekly blended pay: $1,900.00/week - $2,200.00/week
Shift: Monday–Friday, 8:00 AM–5:00 PM, 40 hours/week; occasional on-call required.
Patient Load: 6–8 Medicare Home Health visits per day; Homecare Homebase (HCHB) experience preferred.
Responsibilities: Provide skilled home health nursing, complete assessments (including OASIS), develop and update care plans, administer treatments, educate patients/families, and coordinate care with physicians and the interdisciplinary team.
Clinical Duties: Bedside nursing, symptom management, crisis intervention, family education, palliative/end-of-life care, documentation, and supervision of LPNs/LVNs and home health aides.
Requirements: Minimum 2 years of RN experience (required for PASSPORT patients), active RN license, valid driver's license, automobile insurance, and tablet proficiency.
