Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Medical Records systems.
- Support clients with specialized care needs including Alzheimer's disease, dementia, memory care, and geriatrics by implementing tailored care plans.
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients.
- Provide emotional support and companionship to enhance clients’ quality of life during long-term or hospice care situations.
Skills
- Proven experience working with individuals with developmental disabilities, disabilities, or in assisted living environments.
- Strong knowledge of medical terminology, catheter care, first aid, CPR (Cardiopulmonary Resuscitation), and basic life support techniques.
- Familiarity with home health agency operations, home & community care services, and hospice or end-of-life care practices.
- Ability to perform patient transfers safely using methods such as Hoyer lifts or other transfer techniques; capable of heavy lifting when necessary.
- Excellent communication skills for client interaction and documentation; understanding of medical records management and electronic health records (EHR/EHR systems).
- Knowledge of home health care assistance regulations, HIPAA compliance, and maintaining patient confidentiality at all times.
- Experience working with geriatric patients and those with complex medical conditions like Alzheimer's or dementia.
- Competence in providing personal care assistance while respecting client dignity; ability to work independently or as part of a team in a home setting.
