The Physical Therapist is a professional who evaluates and provides physical therapy services to patients as prescribed by the attending physician.
Partner with the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests, and in establishing goals based on the client’s current level of functioning and potential for improvement.
Guide and instruct patients in prescribed therapeutic activities that are directed towards relieving pain and/or developing or restoring function to maintain maximum performance.
Record and report to the physician and home health office staff, the patient’s reaction to the therapy program and/or any changes in the patient’s condition.
Train patients, families, and/or caregivers in the proper use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, etc.).
Instruct patients, families, and/or caregivers in the client’s total physical therapy program (present them with handouts and instructional content).
Prepare an assessment of the patient’s condition, develop a plan of care and a problem list, write clinical/progress notes, and incorporate everything into the patient’s record.
Prepare periodic evaluations and summary reports of the patient’s condition.
Follow the Plan of Care
Communicate effectively with all those providing care to the patient.
Report to the physician, Director of Nursing and designated Nursing Supervisor.
Notify the Home Health Agency in a timely fashion, of absences due to illness, emergency leave, normal vacation periods, and/or special professional meetings which will affect agreed upon services with the patient/agency.
Participate with staff, patients, patients’ families, and physicians in discharge planning activities and complete the physical therapy discharge summary within the designated framework of Agency policies.
