Scope of Position/Position Summary:
The Care Management Specialist is responsible for coordinating care for an assigned patient population, ensuring efficient, high-quality, and compassionate inpatient or outpatient care. Responsibilities may include coordination across the continuum of care from pre-admission through post-discharge with safe transitions in outpatient settings.
The Care Management Specialist monitors patient progress and care delivery, documenting and communicating key information while leveraging data to improve care timeliness and appropriateness. In collaboration with physicians, nurses, and interdisciplinary teams, the role supports comprehensive assessments, treatment planning, and ongoing evaluation.
This position maintains awareness of emerging state and national healthcare trends and drives initiatives to enhance care quality and ensure effective resource utilization.
Minimum Qualifications:
Bachelors degree in Nursing, Masters preferred. RN licensure; two years recent RN experience applicable to the population required; knowledge of evidenced-based practice and disease specific processes; technology support and payer protocols desired.
