Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to provide comprehensive long-term care services within a specialized healthcare environment. Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
- Administer medications and treatments accurately, including IV infusions, injections, wound suturing, and specialized procedures such as ostomy and catheter care.
- Monitor patients for acute pain management needs and respond promptly to changes in condition with appropriate interventions.
- Coordinate with interdisciplinary teams to develop discharge planning strategies and ensure seamless transitions of care.
- Maintain meticulous medical documentation using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations.
- Provide specialized care for residents with complex needs, including dementia, Alzheimer's disease, developmental disabilities, and memory care requirements.
- Participate in triage activities to prioritize urgent cases and facilitate timely medical responses.
- Support infection control protocols and aseptic techniques during procedures to prevent healthcare-associated infections.
- Assist with diagnostic evaluations such as laboratory specimen collection, radiology procedures like CT scans or cardiac catheterizations, and respiratory support including ventilator management.
- Engage in ongoing education on topics such as hospice & palliative medicine, trauma medicine at Level I or II trauma centers, critical & intensive care practices, and ICD coding updates (ICD-9/10).
- Collaborate with physicians during emergency room or ICU experiences to deliver acute care in high-pressure situations.
- Educate residents and their families about health coaching strategies, medication regimens, and lifestyle modifications tailored for geriatrics or pediatric populations.
Qualifications
- Valid RN license in good standing with the state licensing board.
- Minimum of two years of experience working in long-term care settings such as nursing homes or assisted living facilities.
- Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; experience with medical documentation standards including HIPAA compliance.
- Clinical expertise in areas such as medication administration, IV insertion, tube feeding management, ostomy care, and sterile processing techniques.
- Experience working with diverse populations including seniors, individuals with developmental disabilities, pediatrics (including neonatal resuscitation procedures), and patients requiring behavioral health support.
- Knowledge of advanced nursing procedures such as cardiology-related interventions (e.g., cardiac catheterization), trauma response protocols at Level I/II trauma centers, infectious disease management, and critical care practices.
- Familiarity with coding systems like ICD-9/10 and CPT for accurate documentation and billing processes.
- Strong communication skills for effective collaboration within multidisciplinary teams and engagement with patients’ families.
- Ability to adapt quickly to evolving clinical scenarios while maintaining high standards of patient safety and quality of care.
