Summary
The Care Coordinator plays a vital role in supporting families and clinical teams by managing the intake process, facilitating communication, and ensuring timely completion of evaluation and service paperwork. This position requires strong organizational skills, effective communication, and the ability to multitask in a dynamic, family-centered environment. The schedule required is Monday-Friday 8:30am to 5:00pm. Mostly based out of our Farmington location but will be required to work out of our Glastonbury location 1-2 days out of the week.
Key Responsibilities:
- Answer incoming phone calls promptly and professionally, providing information and support to families.
- Monitor and manage the web inquiries for new referrals.
- Add and track new clients places on waitlists.
- Assist with sending out evaluation packets and intake documents through electronic medical record system for various behavioral health service lines.
- Ensure all patient paperwork is received and completed on time.
- Support score entry for assessments.
- Maintain and update Excel trackers for patient services.
- Greet and assist families and staff in-person at the front window, providing timely and accurate information.
- Collaborate with care coordinators and clinical teams to troubleshoot issues and support client needs.
- De-escalate distressed or concerned families with empathy and professionalism, particularly those new to services or waiting for care.
- Demonstrate phone etiquette, discretion, and clear documentation in all communications.
Qualifications:
- High school diploma or equivalent (required)
- Two years experience in a healthcare, behavioral health, or administrative support setting (preferred)
- One year experience scheduling appointments
- Proficiency in Microsoft Excel, Word, and Outlook
- Experience with CentralReach or similar EMR systems (preferred)
- Strong reading and writing comprehension skills
- Excellent verbal communication and interpersonal skills
- Ability to work both independently and as part of a team
- Strong organizational and time management skills
- Ability to problem-solve under pressure in a fast-paced environment
Preferred Qualifications:
- Familiarity with autism spectrum services and early childhood developmental evaluations
- Bilingual (if applicable)
