About the Role:
As an LVN at our long-term care facility, you will provide essential care to our residents by managing their health needs, administering medications, and coordinating with the healthcare team. Your role includes monitoring patient progress, offering support to residents and families, and ensuring a safe, supportive, and home-like environment.
Qualifications:
- Graduate of an accredited LVN program with a current state license
- Minimum of 2 years of nursing experience in a skilled nursing facility (preferred)
- Proficient in care planning processes and electronic medical records
- Strong attention to detail, documentation, and resident assessment skills
Key Responsibilities:
- Facilitating patient care as directed by the attending physician and supervising nursing activities.
- Assisting with pre-screening, room assignments, and resident discharges.
- Promoting and maintaining high standards of care quality.
- Ensuring a safe and comfortable environment by adhering to safety protocols.
- Conducting interviews, evaluations, and selection of new staff members.
- Communicating any new or unusual findings to relevant parties.
- Upholding confidentiality while fostering positive relationships with residents, team members, and visitors.
- Monitoring and documenting patient health and progress, and implementing care plans.
- Providing education and support to residents and their families about care procedures and health management.
- Participating in quality improvement initiatives and contributing to a collaborative team environment.
- Other duties as assigned by your supervisor.
Physical Requirements:
- Ability to stand and walk for extended periods, with frequent movement throughout the workday.
- Capability to lift, push, and pull weights up to 75 pounds, including moving and repositioning patients and equipment.
- Proficiency in performing repetitive movements, such as bending, stooping, and reaching, while maintaining physical stamina and endurance.
