We're looking for an LPN that is dedicated, compassionate, and reliable individual to join our fantastic team of caregivers at Green Gardens!
Responsibilities:
- Medication Administration and Management
- Communicate with residents' families/POAs about any changes in their plan of care
- Assist with staff training and in-services
- Coordinate with Physicians, Family Members, Physical Therapists, Occupational Therapists and other individuals in the resident care plan
- Audit resident charts
Requirements:
- Vaild LPN license
- Must possess a kind, compassionate and positive attitude
- Must be reliable and professional
- Must have good communication and time management skills
- Current CPR certification
- Preferred experience in dementia setting
