As a Physical Therapist at Holy Trinity, you will help our residents achieve their highest level of function, dignity, and independence. Your daily focus will include:
Evaluate & Plan: Conduct thorough initial evaluations to design personalized, goal-oriented treatment plans tailored to geriatric and long-term care residents.
Deliver Comprehensive Care: Provide hands-on therapeutic interventions, including manual therapy, therapeutic exercise, gait training, neuromuscular re-education, and modalities.
Specialized Geriatric Interventions: Design and implement specialized programs focused on fall prevention, balance improvement, contracture management, and positioning.
Equipment & Assistive Technology: Assess residents' needs for assistive devices, wheelchairs, and orthotics; ensure proper fit, adjustment, and safety training.
Collaborate Broadly: Partner closely with a multidisciplinary healthcare team (OT, SLP, nursing, and restorative aides) to ensure holistic, continuous resident care.
Educate & Empower: Provide ongoing education and training to residents, family members, and nursing staff regarding safe transfer techniques and functional maintenance.
Document Accurately: Maintain precise, timely documentation of patient progress in compliance with state, federal, and insurance guidelines (such as Medicare).
Education: Graduate of an accredited Physical Therapy program (DPT or MPT).
Licensure: Current or eligible state license to practice Physical Therapy.
Clinical Knowledge: Strong foundational knowledge of geriatric rehabilitation, age-related pathologies, and safe handling/transfer techniques.
Skills: Strong clinical reasoning, excellent communication, robust time-management skills, and a patient-centered heart.
Experience: Experienced PTs are highly valued, but passionate new graduates are encouraged to apply! We have the infrastructure to fully support your transition to practice.
