In Home Wellcare is a growing home care agency in Providence, RI, dedicated to providing exceptional, compassionate, personalized care that enhances the lives of our clients, their families, and our team. We are seeking a passionate and dedicated Director of Nursing & Facility Administrator to lead our agency.
In this senior leadership role, you will own our clinical operation and oversee day-to-day administrative functions, ensuring the highest standards of care, quality, and regulatory compliance. You'll lead our nursing team and support the overall operation of the agency. Our current client load is very manageable for one director, with per diem nursing support available as backup when needed. There is also room to grow as we expand our services.

Primary Responsibilities

Complete new patient admissions, initial assessments, and 60-day supervisory visits

Oversee and supervise nurses, CNAs, and homemakers; coordinate per diem visit coverage when needed

Develop and maintain care plans and accurate clinical documentation (Axxess)

Lead policy development, compliance, and survey readiness (RIDOH, EOHHS, CHAP, CMS)

Direct staff orientation, in-service training, and competency evaluations

Oversee administrative operations and support agency growth initiatives and leadership planning
Qualifications

Active RN license in Rhode Island

Nursing leadership or supervisory experience (home care preferred)

Long-term care or skilled nursing facility experience a plus

Strong knowledge of clinical documentation and regulatory compliance

Organized, dependable, and comfortable leading a clinical team and administrative functions
Schedule & Setup

Full-time; flexible schedule, approximately 3 days per week based on agency needs

Company laptop and printer provided; mileage reimbursed for approved travel
