We are seeking an experienced, motivated, and results-driven Home Care Marketer to join our team. This is a field-based position focused on developing referral relationships, generating new patient cases, and expanding the agency's presence throughout the community.
The ideal candidate will have at least 1 year of direct home care marketing experience, a solid understanding of the NYC home care market, and a proven ability to develop referral sources and consistently generate new cases.
This is a performance-driven role, and the successful candidate will be expected to meet established referral and patient acquisition goals. The compensation is also performance-based.
Key Responsibilities:

Develop and maintain relationships with hospitals, physicians, social workers, case managers, rehabilitation centers, senior centers, community organizations, insurance representatives, and other potential referral sources.

Generate qualified patient referrals and new home care cases to meet established monthly performance expectations.

Identify and develop new referral sources and business opportunities throughout assigned territories.

Conduct regular field visits and maintain consistent communication with existing and prospective referral partners.

Educate potential patients, families, and community partners about available home care services and assist in connecting eligible individuals with the appropriate internal team.

Follow up consistently on prospective referrals to support successful conversion into active cases.

Represent the agency at community events, healthcare facilities, networking opportunities, and other outreach activities.

Maintain accurate records of marketing activities, referrals, contacts, and follow-up efforts.

Collaborate with internal departments to facilitate a smooth referral and onboarding process.

Track progress toward established referral and case-generation goals and provide regular activity updates.

Minimum of 1 year of direct home care marketing, community liaison, or business development experience required.

Demonstrated ability to generate patient referrals and successfully bring new cases to a home care agency.

Existing relationships or a book of referral sources within the NYC healthcare or community network is strongly preferred.

Strong understanding of the home care referral and patient enrollment process.
