Occupational Therapist (OT) – PRN
Location: Silver Memories Health Care – Versailles, IN
Silver Memories Health Care is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our rehabilitation team in Versailles, Indiana. This flexible, as-needed position offers the opportunity to make a meaningful impact on residents’ quality of life by helping them achieve greater independence and function in their daily activities.
Evaluate residents and develop individualized occupational therapy treatment plans based on clinical needs and goals
Provide skilled occupational therapy services in accordance with physician orders, professional standards, and facility policies
Assess residents’ functional abilities and recommend adaptive equipment, environmental modifications, and therapeutic interventions
Collaborate with interdisciplinary team members to support optimal resident outcomes
Complete timely and accurate documentation in accordance with regulatory and company requirements
Participate in resident care planning and discharge planning as appropriate
Degree in Occupational Therapy from an accredited program
Current Occupational Therapist license in the State of Indiana
Must be 18 years of age or older
Strong clinical, communication, and documentation skills
Commitment to continuing education and maintaining licensure requirements
