Sundance SADC is seeking an experienced Social Worker to join our team in Brooklyn, NY.
Candidates may be licensed or unlicensed, but clinical social work experience is required.
Responsibilities
- Complete client assessments and psychosocial evaluations
- Develop, review, and update Person-Centered Service Plans (PCSPs)
- Maintain accurate client charts, progress notes, and required documentation
- Identify and monitor clients’ psychosocial and clinical needs
- Coordinate services with staff, healthcare providers, and community resources
- Participate in care planning and interdisciplinary team meetings
- Ensure documentation is completed accurately and on time
- Maintain confidentiality and professional standards
Requirements
- Bachelor’s or Master’s degree in Social Work
- Licensed or unlicensed candidates may apply
- Clinical social work experience is mandatory
- Experience completing PCSPs, assessments, progress notes, and client charts
- Strong documentation, organizational, and communication skills
- Ability to manage multiple client files and meet deadlines
- Legal authorization to work in the United States
- Experience in a Social Adult Day Care, healthcare, behavioral health, or community-based setting is preferred
