Schedule:
- 5 Days/Week
- 7.5 Hours/Day
- 37.5 Hours/Week
- Every Other Weekend Required

Position Overview:
We are seeking an experienced and compassionate Registered Nurse (RN) for an Adult Care/Home Care opportunity serving patients throughout the Bronx Borough. The RN will provide high-quality in-home nursing care, assess patient needs, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.
Responsibilities:
- Assess, plan, implement, and evaluate individualized patient care plans
- Provide direct bedside nursing care to pre- and post-surgical patients
- Administer prescribed medications and treatments accurately
- Perform wound care, infection control, catheter insertion, IV starts, and dressing changes
- Monitor patient vital signs and report adverse reactions promptly
- Operate and adjust specialized medical equipment as needed
- Educate patients and families regarding procedures and post-operative care
- Collaborate with healthcare teams on discharge planning and continuity of care
- Maintain accurate patient documentation and medical records
- Follow established nursing standards and protocols
Requirements:
- Associate Degree in Nursing from an accredited institution required
- Active New York State RN License required
- AHA BLS Certification required
- Minimum 2 years of recent Med/Surg experience required
- Acute care hospital or post-acute/home care experience required
- Most recent experience must be within the past year
- Prior home care experience required
- Must be local to the Bronx/NYC area
