Job Summary
Gateway Healthcare Agency is seeking a dependable, professional, and compassionate Registered Nurse (RN) to join our growing team. The RN will provide clinical oversight for clients receiving home and community-based services and will help ensure that services are delivered safely, appropriately, and in accordance with agency policies, physician orders, plans of care, and applicable state and Medicaid requirements.
This position is ideal for an RN who is organized, comfortable working independently in the community, and experienced with assessments, supervisory visits, care planning, documentation, and caregiver oversight.
Key Responsibilities
The Registered Nurse will be responsible for duties including, but not limited to:
- Conduct initial and ongoing nursing assessments in clients' homes.
- Complete required reassessments and supervisory visits according to the client's program and plan of care.
- Develop, review, and update nursing care plans and service recommendations.
- Assess clients for changes in condition, safety concerns, and changes in service needs.
- Monitor client medications and medication-assistance needs within the agency's scope of services.
- Review medication lists and identify discrepancies or concerns requiring follow-up.
- Provide education to clients, family members, and caregivers as appropriate.
- Supervise and evaluate Personal Care Assistants (PCAs) and other direct-care staff as required.
- Evaluate caregiver competency and compliance with the client's plan of care.
- Provide caregiver education, coaching, and corrective training when necessary.
- Complete timely, accurate, and professional nursing documentation.
- Communicate changes in client condition to physicians, case managers, care coordinators, family members, and agency management as appropriate.
- Participate in incident review, follow-up, and development of corrective or safety measures.
- Assist with quality assurance and compliance activities.
- Ensure nursing services comply with Gateway Healthcare Agency policies and applicable Georgia home-care requirements.
- Maintain client confidentiality and comply with HIPAA requirements.
- Participate in staff training and clinical education as needed.
- Travel to client residences throughout the agency's service area.
Qualifications
Required:
- Current, unrestricted Registered Nurse (RN) license in the State of Georgia
- Graduate of an accredited nursing program
- Current CPR/BLS certification
- Valid driver's license
- Reliable transportation and current automobile insurance
- Ability to travel to client homes
- Strong assessment, documentation, communication, and organizational skills
- Ability to work independently and manage scheduled client visits
- Ability to use electronic documentation systems and basic computer applications
Preferred:
- Previous home health, private home care, community-based nursing, case management, or long-term care experience
- Experience working with elderly individuals and individuals with disabilities
- Experience with Georgia Medicaid waiver programs, including CCSP, SOURCE, and Structured Family Caregiving (SFC)
- Experience supervising PCAs, caregivers, or other direct-care staff
- Knowledge of care plans, supervisory visits, medication assistance, incident reporting, and quality assurance

Compensation
Competitive compensation based on experience and position structure.
Compensation may be structured as hourly, salary, or per-visit depending on the RN's assigned responsibilities and work schedule.
Mileage/travel arrangements will be discussed during the interview process.
Schedule
Schedule may include:
- Monday through Friday
- Flexible daytime scheduling
- Scheduled client assessments and supervisory visits
- PRN/Part-Time opportunities may be available
