Physical Therapist (PT) – Home Health PRN / Part-Time (ASAP Need) – Immediate Visits Available

Therapy Partners of Texas is seeking a motivated and dependable Physical Therapist to provide home health visits for our contracted home health agency partners throughout Sherman, Melissa, Anna, Denison, and surrounding North Texas areas. (Service area is within 30 miles of our Sherman office.)
This is an excellent opportunity for a Physical Therapist looking to supplement income with a flexible PRN/Part-Time schedule while working with an experienced home health team. Clinicians will provide services to patients referred through our contracted home health agency partners.
Responsibilities
- Perform Physical Therapy evaluations, reassessments, and routine visits in the patient's home
- Develop individualized treatment plans
- Collaborate with physicians, nurses, and interdisciplinary team members
- Complete documentation accurately and timely
- Participate in patient care coordination as needed
- Communicate with agency clinical teams regarding patient progress and needs
Qualifications
- Current Texas Physical Therapist License
- Home Health experience preferred
- Knowledge of Medicare documentation requirements - ability to complete notes within a 48-72 hour timeframe
- Reliable transportation and current automobile insurance
- Strong communication and organizational skills
- WellSky/Kinnser experience preferred but not required
