POSITION SUMMARY:
This position is responsible for providing patient care in a compassionate, professional, ethical, and knowledgeable approach. This includes, but is not limited to assessment, planning, education, prevention, implementation and evaluation of the patients? status, plan of care and goal achievement.
ESSENTIAL JOB DUTIES:
- Educates the patient and their families on their current condition, including dietary and lifestyle changes, medication therapy and exercise.
- Develops patient care plan in order to monitor and maintain patient condition.
- Performs adequate medication analysis and adjustments based off testing as necessary.
- Continually assesses patient care plan and adapts plan to meet patient needs.
- Collaborates with appropriate leadership in order to establish best plan of care for patients.
- Follows policy when disposing of biohazardous materials, labeling, and handling of all materials.
- Demonstrates a working knowledge of cardiac nursing theory, techniques, principles, and practice in order to care for the patient.
- Maintains working knowledge of all medical equipment.
- Participates in interdisciplinary team meetings and appropriate facility meetings.
- Travel to other offices based on patient care needs.
- Other duties as assigned per the need of the organization and within scope of licensure.
