Job Summary
We are seeking compassionate and dedicated individuals to join our team as Direct Support Professionals (DSP) and Family Model Providers (FMP). In this vital role, you will provide personalized care and support to individuals with developmental and intellectual disabilities, including those with autism, neurodevelopmental disorders, and cognitive disabilities. Responsibilities
- Assist individuals with daily living activities (ADLs), including grooming, hygiene, meal preparation, and mobility support
- Implement behavior management strategies tailored to each individual’s needs to promote positive behaviors and reduce challenging behaviors
- Support communication efforts using techniques such as American Sign Language or other augmentative communication methods when applicable
- Provide transportation assistance using a standard passenger car for appointments, community outings, or errands
- Collect data on progress and behaviors to inform care plans and report changes to supervisors or family members
- Administer medication following prescribed guidelines and document all medication-related activities accurately
- Foster social skills development through engaging activities, recreational outings, and community participation
Requirements
- Experience working with individuals with autism, developmental disabilities, or neurodevelopmental disorders is highly preferred
- Knowledge of special education principles and behavioral health practices
- Ability to operate a standard passenger car safely and reliably for client transportation needs
- Strong communication skills for effective client interaction and collaboration with families and team members
- Certification in First Aid and CPR is required; additional training in behavior management or medication administration is a plus
- Home & community care experience demonstrating caregiving skills in diverse settings
- Familiarity with assisting clients with ADLs, including personal hygiene, feeding, mobility, and medication management
- Ability to work independently as well as part of a team in a dynamic environment

This independently contracted position will provide a 24/7 family-like setting for an individual.

We are seeking someone who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to develop a comfortable relationship with the person you support.
REQUIREMENTS:
- Valid Driver's License
- High School Diploma/GED
- Previous experience working as a Direct Support Professional is preferred
- Will need to complete all required training per DDA/ECF requirements through CHS Group TN, LLC.
- Must be open to monthly supervisory visits
Our Host Home Providers are contracted and reimbursed at a per-diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
