As a Physical Therapist, you will provide individualized rehabilitative care to patients recovering from illness, injury, or surgery in the comfort of their homes. Responsibilities include:

Perform and document comprehensive initial evaluations and assessments

Develop and implement personalized treatment plans to improve mobility, strength, balance, and function

Monitor patient progress and modify treatment plans to maximize outcomes

Educate patients and families on exercises, safety, and ongoing care needs

Maintain accurate and timely clinical documentation

Collaborate with physicians, caregivers, and interdisciplinary healthcare teams

Follow infection control and safety protocols at all times

Support the agency’s mission of delivering compassionate, high-quality patient care

Graduate of an accredited Physical Therapy program
-
Current state Physical Therapist license
-
CPR Certification required
-
Valid driver’s license and auto insurance
-
Home health experience preferred

HomeCare HomeBase experience preferred

Ability to stand, walk, bend, kneel, squat, and climb for extended periods

Ability to lift, carry, push, or pull up to 100 lbs.
