Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ conditions, noting any changes or concerns related to vital signs, behavior, or health status
- Administer medications and treatments following medical instructions and home healthcare regulations
- Support patients with specialized needs including Alzheimer's care, dementia care, end-of-life care, and hospice services
- Operate Electronic Health Record (EHR) systems to document patient information accurately and securely in compliance with HIPAA standards
- Use medical terminology appropriately when recording observations or communicating with healthcare professionals
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment for clients
- Utilize equipment like Hoyer lifts or other assistive devices safely to support patient mobility and prevent injury
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred
- Demonstrated experience caring for geriatric patients or individuals with disabilities in a home or community setting
- Knowledge of medical documentation procedures and familiarity with EMR/EHR systems
- Hands-on experience providing dementia or memory care is highly valued
- Certification in CPR and Basic Life Support (BLS) required; First Aid certification is a plus
- Experience assisting with catheter care, medication administration, vital signs monitoring, and hospice/end-of-life care preferred
- Strong understanding of home health regulations and patient privacy laws such as HIPAA
