In this vital role, you will provide comprehensive support to clients in their own homes or community settings, promoting independence, safety, and well-being.
Schedule: Mon-Fri 8a-11a, Mon and Thu 2p-5p
Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity.
- Monitor patients’ health by observing vital signs, medication administration, and overall condition, reporting changes promptly to healthcare professionals.
- Support clients with mobility and transfers using manual handling techniques and equipment such as Hoyer lifts or patient transfer methods to promote safety.
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to support health goals.
- Provide transportation assistance for appointments, errands, or community activities, ensuring safe and reliable client mobility.
- Maintain detailed care plans and documentation in compliance with HIPAA regulations, ensuring confidentiality and accuracy of patient records.
- Engage in patient observation and communication to assess emotional well-being, especially when caring for individuals with Alzheimer’s disease, dementia, autism, or other neurodevelopmental disorders.
Qualifications
- Must Have: Experience with caring for Type 1 Diabetics
- Must Have: Experience with caring for individuals with dementia or related disorders
- Proven experience working with individuals with seniors in settings such as group homes, assisted living facilities, nursing homes, or home health environments.
- Knowledge of patient care techniques including medication administration, vital signs monitoring, and Diabetic Care
- Familiarity with specialized care such as Alzheimer’s care, memory care, hospice care, or dementia support is highly desirable.
- Ability to perform heavy lifting and manual handling safely while assisting with transfers or mobility needs.
- Strong communication skills for effective client interaction and collaboration with healthcare teams.
- Experience working with individuals with autism or neurodevelopmental disorders is a plus.
- Valid driver’s license and reliable transportation for client outings or appointments preferred.
Pay: $21.00 per hour
Expected hours: 21.0 per week
