Job Overview
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to provide exceptional, patient-centered care in a home health setting. 7AM to 3PM , 3P to 11PM, 11PM to 7AM.
This position involves caring for a 19-year-old female patient who requires total care assistance, including management of a gastrostomy tube (G-tube), tracheostomy, and mechanical ventilator. Patient is 100 to 115 lbs. Bed Bath or shower chair. Family members will help at times. Once The LVN will be responsible for monitoring the patient's condition, administering treatments as prescribed, maintaining accurate documentation, and ensuring a safe and comfortable home environment. Under the supervision of RN Case Manager. Morning shift , Mid day shift and Grave Yard open.
Responsibilities

Traininig Orientaion Shadowing will be provided.
- Provide comprehensive nursing care for a medically complex patient in the home setting.
- Manage and monitor G-tube feedings, tracheostomy care, and ventilator support.
- Assess and document patient status, vital signs, and any changes in condition.
- Administer medications and treatments according to physician orders.
- Collaborate with family members and the healthcare team to ensure continuity of care.
- Maintain a safe, clean, and supportive environment while promoting patient comfort and dignity.
Qualifications
- Current and active California Licensed Vocational Nurse (LVN) license.
- Home Health Experienced nurse is preferred , pediatric, or long-term care settings.
- Experience with tracheostomy, ventilator, and G-tube care preferred.
- Strong clinical assessment, communication, and documentation skills.

Pay: $27.00 - $30.00 per hour
