Dedicated Regional Nurse Coordinator Support
Each provider is supported by a dedicated Regional Nurse Coordinator who serves as an extension of the provider's clinical team.
Regional Nurse Coordinators assist with:
- Daily scheduling and care coordination
- Documentation and note management
- Facility communication
- Prescription refills and medication-related requests
- Forms, orders, and administrative tasks
- Communication with nursing staff, families, pharmacies, and other healthcare partners
This model reduces administrative burden, improves communication, and allows providers to spend more time focused on patient care.

Our providers typically see approximately 20 patient visits per day, allowing adequate time for thoughtful assessments, meaningful patient interactions, and high-quality clinical decision-making.

Compensation & Benefits
We offer a competitive compensation package designed to reward experience, quality care, and provider success.
Compensation
Base Salary: $110,000–$125,000 annually (Full-Time)
Compensation is based on experience, qualifications, and schedule.
Total Compensation Package: Approximately $133,000–$150,000+ annually

Position Responsibilities
The CRNP will provide comprehensive primary and chronic care management services to residents in Skilled Nursing, Assisted Living, and Independent Living communities.
Responsibilities include:
- Performing patient assessments and developing treatment plans
- Managing acute and chronic medical conditions
- Collaborating with physicians, nursing staff, facilities, families, and healthcare partners
- Providing evidence-based, patient-centered care
- Participating as part of a collaborative interdisciplinary care team
Qualifications
- Current Pennsylvania Certified Registered Nurse Practitioner (CRNP) license
- Current DEA registration or ability to obtain
- Strong clinical assessment and communication skills
- Ability to work independently while collaborating within a team environment
- Commitment to providing compassionate care to older adults
Experience in geriatrics, long-term care, family medicine, internal medicine, or primary care is preferred but not required.
