About ProMedix Health
ProMedix Health is a virtual care platform delivering chronic care management programs to Medicare patients in partnership with primary care physician groups and healthcare systems. We combine clinical staffing, operational infrastructure, and proprietary technology to monitor patients remotely and provide continuous care management outside of traditional office visits. Our programs predict health decline prior to observable symptom or physical change. We are in an active growth phase expanding toward a national footprint.
The Role
We are looking for an experienced Director of Nursing to lead the clinical team at ProMedix Health. This is a hands-on nursing leadership role responsible for the performance, quality, and compliance of our clinical staff as they deliver virtual chronic care management to a Medicare patient population.
The right candidate is an RN with genuine nursing leadership experience - someone who has supervised clinical teams, held staff accountable to documentation and care standards, and operated in environments where CMS compliance and patient outcomes are non-negotiable. You do not need to come from a telehealth background, but you need to be tech-comfortable and genuinely interested in virtual care delivery. Strong experience in SNF, acute care, or outpatient chronic disease management translates well to this environment.
Experience in RPM, CCM, or virtual care delivery is a significant plus but will not disqualify an otherwise strong nursing leader who brings the right clinical foundation.
What You Will Do
Clinical Team Leadership
- Lead, supervise, and develop a team of RNs, LVNs, MAs, and Care Coordinators delivering virtual chronic care management to enrolled Medicare patients
- Set productivity standards and hold the team accountable - panel capacity, patient engagement rates, documentation completion, and care management time per patient
- Recruit and onboard clinical staff as the program scales
Clinical Quality and Compliance
- Own clinical documentation standards - ensure all patient encounters clearly document medical necessity and support compliant billing
- Conduct regular internal audits of clinical records, identify deficiencies, and drive remediation to closure
- Train clinical staff on documentation expectations that produce defensible records and support billed services
- Coordinate with revenue cycle and billing partners to ensure documentation consistently supports all submitted claims
- Maintain audit-ready records at all times across all programs
- Become a platform super-user across our proprietary virtual care portal and EHR system - serving as the clinical team's first resource for workflow questions, documentation standards, and platform issue escalation
Care Team and Physician Coordination
- Serve as the primary liaison between clinical staff (RNs, LVNs, MAs, Care Coordinators) and ordering physicians and PCPs - ensuring clear, timely communication on patient status, care plan changes, and clinical escalations
- Translate clinical findings from the care team into actionable information for ordering physicians and ensure physician orders and care directives are clearly communicated back to the team
- Maintain defined protocols for when and how the care team escalates to ordering physicians and hold staff accountable to those protocols
Patient Monitoring and Alert Management
- Oversee the clinical response to RPM device alerts - establish and enforce response time standards
- Ensure monitoring workflows are followed consistently and that alert escalation protocols are clearly defined and practiced
- Monitor patient engagement across the enrolled population and take action where engagement falls below defined thresholds
SOPs, Credentialing, and Reporting
- Maintain and improve clinical SOPs for patient onboarding, monitoring, care plan development, escalation, and disenrollment
- Ensure all clinical staff licenses, credentials, and certifications remain current with no lapses
- Deliver a weekly clinical performance report to leadership covering key metrics, variances, and recommended actions
What We Are Looking For
Required
- Active RN license in California, in good standing
- BSN required - MSN preferred
- Minimum 5 years of nursing leadership experience including direct supervision of RNs and clinical staff
- Clinical background in SNF, acute care, outpatient chronic disease management, or a comparable high-acuity environment
- Strong working knowledge of CMS documentation standards and clinical compliance requirements
- Demonstrated ability to hold a clinical team accountable to productivity, documentation, and quality standards
- Comfort with technology platforms and genuine interest in virtual care delivery
Strong Plus - Not Required
- Experience in RPM, CCM, CoCM, TCM, or other chronic care management programs
- Prior telehealth or virtual care delivery experience
- Familiarity with EHR systems in a care management or value-based care context

- Candidates without direct nursing staff supervision experience
Compensation and Benefits
$140,000 - $165,000 annually, based on experience and qualifications

This is a 100% on-site role based in Santa Ana, CA. Remote and hybrid arrangements are not available.
