JOB
This position is responsible for providing direct and indirect case management services to Sixth Judicial District Specialty Court participants. The responsibilities of this position include conducting individual case management sessions with participants, documenting activities, connecting participants with community resources, as well as other duties to support the program functioning. Performs a variety of technical tasks relative to assigned areas of responsibility, such as data entry, statistics, grants, preparation of reports, etc. Reports to the Specialty Courts Coordinator.
DISTINGUISHING CHARACTERISTICS:
This position is distinguished by the following responsibilities: (1) Providing direct case management services to program participants; (2) Data entry and tracking; (3) Program support and coordination; and (4) Liaison between court and community providers. This position reports to the Specialty Courts Coordinator. The Specialty Courts Case Management position is funded in whole or in part by grant funds and not by Humboldt County appropriations and may terminate when funding is no longer available. Continued employment beyond this time will be contingent upon satisfactory performance, compliance with Humboldt County policies and procedures, the continued availability of funds, and the needs of the department.
EXAMPLE OF DUTIES (Performance of these functions is the reason the job exists. Assigned job tasks/duties are not limited to the essential functions).

Completes comprehensive case management assessments and ongoing case management services to Specialty Court participants in order to connect them to the services necessary to meet their individualized needs.

Assists participants with habilitative services, housing and financial planning; provides support to encourage positive choices for the success of the individual.

Stays up to date with and educates clients on resources available.

Creates individualized case plans based on Ohio Risk Assessment System (ORAS)

Learns and implements Adult Drug Court Best Practice Standards and other standards relevant to specialty court models.

Collects and maintains data related to program and participant activities for purposes of care coordination and program evaluation.

Attends and actively participates in staffing and court hearings to update the Specialty Team on participant activities and progress.

Develops and maintains cooperative working relationships with members of the Specialty Courts Team including, but not limited to, District Attorney, Defense Counsel, Probation, treatment providers, law enforcement, and other community agencies.

Attends required conferences, training, and committee meetings.

Assists in coordinating training for Specialty Courts.

Demonstrates a comprehensive understanding of and complies with all applicable confidentiality and privacy laws related to substance use and mental health treatment, including 42CFR, part 2 and 45 CFR (HIPAA).

Represents the Sixth Judicial District Court and Humboldt County with dignity, integrity, and a spirit of cooperation in all relationships with staff, clients, and members of the public.

Prepares all paperwork for staffing and court sessions in a timely manner and as directed.

Develops case management program with tracking and statistical analysis components required by federal grant mandates.

Creates and maintains accurate files on Specialty Court participants’ progress through programming.

Assists with applications, processing and transportation arrangements for participants admitting to withdrawal management and residential treatment services.

Performs other duties as assigned.
