- Schedule: Monday–Friday | 8:30 AM – 5:00 PM
- Employment Type: Full-Time
As an RN Case Manager, you will coordinate and oversee the care of a dedicated patient caseload while providing skilled nursing services in the home.
Responsibilities include:
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing visits and case management services.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Monitor patient progress and adjust care plans as needed.
- Provide patient and family education to promote independence and improved health outcomes.
- Complete timely and accurate clinical documentation.
- Supervise and collaborate with interdisciplinary team members to ensure exceptional patient care.
- Advocate for patients while promoting quality, safety, and continuity of care.

- Current Registered Nurse (RN) license in the State of New Hampshire
- Minimum of two (2) years of skilled nursing experience preferred
- Previous nursing supervisory experience preferred
- Recent wound care experience preferred
- Current CPR Certification
- Reliable transportation and valid driver's license
- Strong assessment, communication, organizational, and clinical decision-making skills
