Hospice Admissions Nurse (RN)

We are seeking a full-time Hospice Admissions Nurse to complete hospice admissions and support patients and families during the transition into hospice care. This role serves hospice patients receiving services in their own homes or facility settings. Normal operating hours are Monday through Friday, 8:30 AM to 4:30 PM.

Position Summary
- Completes hospice admissions by assessing eligibility, educating patients and families, and facilitating a smooth transition into hospice services.
- Serves as a key clinical liaison between referral sources, patients, families, and the interdisciplinary team.
Key Responsibilities
- Conduct hospice admissions in private homes or facility settings.
- Assess patient condition, hospice eligibility, and appropriateness for services.
- Educate patients and families on hospice philosophy, services, and expectations.
- Obtain required consents and complete admission documentation accurately and timely.
- Collaborate with referral sources, physicians, and internal team members to coordinate admissions.
- Communicate admission details and clinical findings to the interdisciplinary team.
- Document all admission activity in HCHB (Homecare Homebase).
- Travel within the assigned service area to complete admissions.
- Active RN licensure in the state of practice.
- One year of nursing experience required; hospice and/or admissions experience preferred.
- Strong communication skills and comfort leading sensitive conversations.
- Ability to work efficiently in time-sensitive situations.
- Reliable transportation for home and facility visits.
- EMR experience preferred; HCHB familiarity a plus.
