We are seeking a dedicated, organized, and compassionate Patient Care Coordinator to join our home health care team. The Patient Care Coordinator will play a key role in coordinating patient services, supporting patients and caregivers, and ensuring effective communication between patients, healthcare providers, caregivers, and the office.
The ideal candidate is bilingual in English and Spanish, highly organized, tech-savvy, detail-oriented, and comfortable working in a fast-paced, all-digital healthcare environment. This position requires excellent communication skills and the ability to manage multiple priorities while maintaining a high level of professionalism and confidentiality.
Key Responsibilities
- Coordinate and schedule home health services based on patient needs, care plans, and staff availability.
- Serve as a primary point of contact for patients, families, caregivers, and healthcare providers.
- Register new patients and complete required intake and demographic information according to established procedures.
- Verify and maintain accurate patient information, including addresses, telephone numbers, insurance, and billing information.
- Maintain accurate and up-to-date electronic patient records and documentation.
- Coordinate communication between patients, caregivers, physicians, nurses, therapists, and other healthcare professionals.
- Obtain medical records, physician orders, referrals, reports, and other documentation as requested.
- Follow up on physician orders, authorizations, referrals, and other documentation necessary to coordinate patient services.
- Answer incoming calls, respond to patient and caregiver inquiries, and provide timely assistance.
- Address patient and family concerns professionally and escalate issues when appropriate.
- Assist with coordinating caregiver schedules, service changes, call-outs, and coverage to help ensure continuity of care.
- Monitor scheduled services and communicate any changes or concerns to the appropriate team members.
- Assist with maintaining compliance with company policies, healthcare regulations, and documentation requirements.
- Protect patient confidentiality and handle sensitive health information in accordance with HIPAA requirements.
- Utilize computers and office technology for electronic documentation, scheduling, email, word processing, spreadsheets, scanning, printing, and other administrative functions.
- Maintain a professional, courteous, and patient-centered approach in all interactions.
- Perform other administrative and coordination duties as assigned.
Qualifications & Skills
- Bilingual in English and Spanish required.
- Previous experience in home health, healthcare administration, patient coordination, medical office, or a related field preferred.
- Strong computer and technology skills; must be comfortable working in an all-digital, paperless environment.
- Experience with electronic medical records (EMR/EHR) and scheduling systems is a plus.
- Excellent verbal and written communication skills.
- Strong customer service and interpersonal skills.
- Excellent organizational skills with the ability to multitask and prioritize responsibilities.
- Strong attention to detail and accuracy in patient documentation.
- Ability to work independently while also collaborating effectively with a healthcare team.
