Job Summary
Life Care Center of Fort Wayne is seeking dedicated and skilled Licensed Practical Nurses (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, medical documentation, and emergency response, with a focus on delivering compassionate support to residents. As an LPN, you will play a vital role in providing quality nursing services, ensuring compliance with health regulations, and supporting the overall well-being of our residents through effective nursing practices.
Duties
- Administer prescribed medications and treatments accurately while monitoring residents’ responses.
- Assist in developing and implementing individualized care plans based on residents’ needs and medical conditions.
- Document all patient interactions and medical information in electronic health records (EHR) with precision and confidentiality.
- Provide basic life support (BLS) and first aid as needed during emergencies or routine care situations.
- Collaborate with interdisciplinary teams to coordinate patient care, including triage and disease prevention strategies.
- Ensure compliance with health regulation policies such as HIPAA and maintain the confidentiality of sensitive information.
- Support chronic disease management by monitoring vital signs, administering therapies, and educating residents on health maintenance.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) in the appropriate state or jurisdiction.
- Proven experience in acute care settings or long-term care facilities demonstrating proficiency in patient care.
- Knowledge of medical terminology, physiology, anatomy, and nursing best practices.
- Familiarity with medical software systems such as EMR/EHR for accurate documentation.
- Certification in CPR (Cardiopulmonary Resuscitation) and basic first aid required; additional certifications in emergency care preferred.
- Understanding of health regulation policies including HIPAA compliance and medical records management.
- Strong communication skills with the ability to handle confidential information professionally.
- Prior leadership or nursing supervisory experience is advantageous but not mandatory; a commitment to ongoing professional development is essential.
