Responsibilities:
* Supervise all patient care services and staff (RN, LPN, OT, PT, HHA)
* Coordinate patient care and referrals
* Ensure ongoing assessment of patient needs
* Provide direct patient care when staffing requires
* Support clinicians with OASIS, documentation and case management
* Review clinical documentation
* Lead case conference, clinical meetings, and in-services
* Ensure patient charts and records are accurate, and regulatory compliant
* Monitor clinical outcomes
Qualifications:
* Registered Nurse (RN) with current state license
* 2-3 years of home health experience required
* Strong leadership, communication, and problem-solving skills
* CPR Certification, valid driver's license and auto insurance
Pay: $102,985.70 - $124,025.78 per year
