Job Summary
Part-time RN to provide nursing oversight across a Wheeling CILA home. This is a 5-10 hour/week position with flexible daytime hours Monday-Friday, 1-2 evening visits per week for staff competency and resident needs, and on-call phone triage- 30-minute response. No heavy admin — mostly home visits, MD coordination, and DSP staff training/competency. Ideal for an I/DD, CILA, home health, or community nurse who likes autonomy, relationships, and person-centered care.
What You'll Do
Nursing health assessments (initial, annual, and as-needed) for CILA residents; document in EHR
Develop and update person-centered Health Care Plans and support ISP outcomes — HCBS Settings Rule compliant
Manage physician orders, labs, medication refills, appointment packets, and hospital/ER discharge follow-up within 24 hours
Rule 116 medication oversight: MAR review and reconciliation, med counts, storage, documentation, error reporting
Train and assess DSP competency per 59 IAC 116 / Rule 116
On-call phone triage (30-min response, shared rotation) for health changes, incidents, and staff questions
Infection control, OSHA universal precautions, and prevention; coordinate immunizations and health screening
OIG Rule 50 critical incident awareness and reporting, QA, HIPAA-compliant documentation
Collaborate closely with QIDP, House Manager, DSP's, families/guardians, and community providers
Support community integration and choice — health services that enable, not restrict, community life
Requirements
Active Illinois RN license (required, in good standing) – IL Nurse Practice Act 225 ILCS 65
BLS/CPR certification (AHA or equivalent)
Able to pass: Illinois background checks - CANTS (child abuse), LEID, OIG LEIE/SAM exclusion, HFS Sanctions List, Illinois Sex Offender Registry, BSP fingerprint-based check if required for site
1+ year I/DD, CILA, home health, community nursing, or long-term care preferred
Rule 116 Medication Administration Trainer certification preferred (RN)
Knowledge of: 59 IAC 115, 59 IAC 116, 59 IAC 120, OIG Rule 50, HCBS Settings Rule 42 CFR 441.301, HIPAA
Organized, reliable, person-first communication, able to teach DSP's with patience and clarity
Pay & Schedule
$38-$42/hr, Rule 116 Trainer certification, and CILA/I-DD experience. This is a W-2 part-time role at 5-10 hrs/week, Monday-Friday flexible daytime hours. 1-2 evenings/week for staff training or resident need. Weekend only for urgent issues — otherwise no weekend requirement.
Part-time benefits: PTO *Eligibility per hours/policy. At-will employment.
Pay: $38.00 - $42.00 per hour
