Advanced Senior Care (ASC)
We are an experienced primary care provider of long term care medical services in the SNF, ALF, RCF & ILF environments. We are growing and expanding to meet the needs of elderly and chronically ill patients in mid-Missouri. We provide primary care services for permanent residents in LTCF's, as well as acute/post-acute rehab patients as they rehabilitate to their previous level of care and function.
Our practice model accommodates a variety of clinician practice styles and allows for a very high degree of flexibility and autonomy.
We are able to provide excellent continuity of care while fostering close collaborative relationships with our patients and their caregivers.
We provide administrative, LPN and mid - level provider support for our providers, and embrace a highly mobile and paperless practice.

Pay: $115,000.00 - $135,000.00 per year
