Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our dynamic Home Health team. Responsibilities
- Deliver comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Cerner or Epic.
- Administer medications and injections accurately following aseptic techniques and hospital protocols, including IV infusions and medication management for acute pain or chronic conditions.
- Perform specialized procedures such as suturing, catheterization, tube feeding, and wound care while maintaining sterile environments.
- Manage complex patient cases in various settings including ICU, emergency room, long-term care facilities, hospice, or outpatient clinics with expertise in trauma medicine, infectious disease care, or geriatrics.
- Collaborate with physicians and healthcare professionals to develop and implement individualized care plans aligned with NCQA standards and ICD coding requirements.
- Conduct triage assessments in urgent care or emergency medicine environments to prioritize patient needs efficiently.
- Maintain accurate medical documentation, ensure HIPAA compliance, and update electronic health records (EHR/Epic) meticulously for seamless continuity of care.
Skills
- Proven experience working with diverse patient populations including pediatrics, geriatrics, individuals with disabilities, or those requiring memory or dementia care.
- Proficiency in advanced nursing procedures such as ventilator management, dialysis support, cardiac catheterization assistance, or neonatal resuscitation procedures.
- Familiarity with EMR/EHR systems like Cerner or Athenahealth for effective documentation and medical records management.
- Strong knowledge of physiology, anatomy, and medical terminology to perform accurate patient assessments and diagnostic evaluations.
- Ability to handle high-pressure situations in trauma centers (Level I & II), critical care units (ICU), or emergency rooms with expertise in trauma medicine and acute care protocols.
- Experience working within hospital medicine settings including surgical units or outpatient clinics; knowledge of ICD-10/ICD-9 coding is advantageous.
- Excellent communication skills for patient education on health coaching topics such as medication administration or discharge planning; adept at specimen collection/processing when needed.
