Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.

Provide leadership, supervision, and support to team of assigned
staff that are proving services to individuals experiencing homelessness, involved in the court system, and to consumer with severe and persistent mental illness.
Job Title: Care Management Supervisor, Homeless Services (Nashville, TN)
Schedule: Monday to Friday, 8:00 AM-4:30 PM
Salary: $53,500 (base pay) with additional incentive for language skills, leadership, and behavioral health experience. Mileage reimbursement available.
JOB SUMMARY: Provide leadership, supervision, and support to team of assigned staff that are proving services to individuals experiencing homelessness, involved in the court system, and to consumer with severe and persistent mental illness.
RESPONSIBILITIES:
- Supervise care management and outreach teams and ensure consumers are seen according to agency standards and grant requirements and provide appropriate clinical and administrative recommendations.
- Ensure program protocols/policies and procedures are carried out.
- Conduct planning session and meeting with team as required by Agency and as needed.
- Coordinate prompt assignments of consumer to care mangers and ensure appropriate contact.
- Promptly address team concerns and work to obtain resolution.
- Responsible for on-site consulting with Office Coordinator, Program Manager, and Care Managers as needed or assigned.
- Provide support and consultation for care managers with consumers in crisis situations.
- Ensure consumers are seen upon admission and discharge from the hospital and respite within Agency and BHO standards.
- Ensure consumer’s daily psychiatric, medical, and environmental needs are planned for and coordinated.
- Review care management documentation to ensure documentation is individualized and meets consumer’s needs and show medical necessity for continued service.
- Ensure consumers are provided efficient transitions within the Agency and into the community.
- Assist care manager with referral and linkage to community resources and advocate for consumers when necessary.
- Meet regularly with Program Manager for supervisions (individual and/or group).
- Attend regular Supervisor’s meeting.
- Attend and facilitate daily and weekly team meetings and ensure multi-disciplinary approach. Ensure appropriate documentation for these meetings.
- Counsel care manager on performance and complete scheduled performance appraisals.
- Collect and monitor completion of clinical documentation as outlined in Agency policy and procedures, (i.e., hospital, 90-day housing review, etc.)
- Collect and review all appropriate documentation for payroll and financial reimbursement and submit to Accounting Department in a timely manner.
- Interpret necessary agency reports; use them to monitor production standards of assigned care managers/outreach specialist as outlined in the Agency policy and procedure manual and per grant requirements.
- Provide individual supervision of assigned Care Managers and Outreach Specialist and maintain supervision records in accordance with Agency and BHO standards. Supervision includes but not limited to in-office meetings, ride-alongs, and chart audits.
- Assist in the hiring of care management staff.
- Ensure timely and appropriate documentation as required by the Agency or grantee, including the use of HMIS database.
- Develop and maintain community relations through prompt follow-up with contacts.
- Participate in community-coordinated events and initiatives put in place by the Metro Nashville Continuum of Care, including community meetings required or necessary for grant participation.
- Assist Program Manager in the completion of quarterly and annual reports for PATH Program and agency reports.
Attend and assist in site monitoring visits.

SKILLS AND EXPERIENCE:
Master’s degree (M.A., M.S.) in a health-related field of counseling, psychology, or social work preferred.
Licensed or license eligible preferred.
Experience in a mental health setting preferred.
Two years of supervisory experience in a mental health setting.
