Overview
We are seeking a dedicated and highly skilled Registered Nurse to join our healthcare team. The ideal candidate will possess a comprehensive understanding of patient care, medical procedures, and healthcare documentation, with a strong commitment to delivering exceptional nursing services. This role offers an opportunity to work in a home health setting. The Registered Nurse will play a vital role in ensuring patient safety, comfort, and recovery through expert assessment and compassionate care.
Responsibilities
Perform Admission of patients into home health, Re-certification of care, Resumption of care and Supervisory visits and Regular RN visits, Wound care, IV infusion and Quality Assurance.
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine individual care needs.
- Administer medications and injections accurately following prescribed protocols while ensuring proper documentation in electronic health records (EHR) systems such as cradlemrx.
- Perform specialized procedures including IV infusion, catheterization, tube feeding management, and sterile wound dressing changes with strict adherence to aseptic techniques.
- Monitor patients’ physiological status in various settings such as home health.
- Collaborate with multidisciplinary teams to develop and implement personalized care plans aligned with patient conditions and healthcare standards like HIPAA compliance.
- Provide education and health coaching to patients and their families regarding medication administration, disease management—including infectious disease care—and post-discharge instructions.
- Ensure accurate medical documentation of all patient interactions, assessments, procedures, and treatments in accordance with hospital policies and coding standards such as ICD-10 or CPT coding.
- Participate in quality improvement initiatives focused on infection control, and other programs to enhance patient outcomes.
Qualifications
- Current Texas State Board of nursing license
- At least one year of clinical nursing experience required - One year Home Health Care Experience is a plus but not required.
- The clinician must have reliable transportation with a current valid driver’s license, current auto insurance,
- Current CPR, TB screen done in less than one year.
- The nurse must have excellent interpersonal skills, strong organizational skills and be detail oriented.
- Proven experience working across diverse clinical environments including hospital medicine, post-acute settings such as hospice or home care.
- Demonstrated proficiency with EMR/EHR systems like cradlemrx for accurate documentation and order entry.
- Strong knowledge of nursing practices related to medical-surgical care, and geriatrics.
- Experience performing complex procedures such as IV infusion, venipuncture, phlebotomy, ostomy care, or gastrostomy tube management and wound dressing changes.
- Familiarity with healthcare regulations including HIPAA compliance and OSHA standards for infection control.
- Excellent communication skills combined with critical thinking abilities to assess patient needs effectively and coordinate multidisciplinary interventions.

This job can be a full-time position @ 40 hours per week from Monday to Friday 8:00 am to 5:00 pm. Per visit or PRN position available.
