Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
The SUD Case Manager works closely with adult clients diagnosed with SUD to assure their daily needs are met. They connect clients to services both within and outside of Hegira Health.

EDUCATION AND EXPERIENCE REQUIREMENTS:
- Bachelor’s Degree, specializing in social sciences, psychology, social work, counseling or related field.
- Possess a CAADC, CADC or file for a development plan within 30 days of employment.
- Provided direct treatment services, including SUD, to adults in a behavioral health setting for one year or more.
- Knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders.
- Knowledge of diagnostics ,psychopharmacology, and supportive treatment approaches as applied to the SUD adult population.
- Possess a valid Michigan chauffeur's license or obtain during the hire process.
RESPONSIBILITES:
Intake
- Completion of the Case Management Assessment within 1 week of intake appointment.
- Make recommendations regarding case management services.
- Case Management Assessment is updated annually, at minimum.
- Identify issues that may exist regarding safety, natural supports, and community involvement.
- Identify referrals needed for treatment.
- Educate client and/or guardian with respect to admission information.
- Complete Urine Drug Screen.
- Schedule appointment for psychiatric evaluation.
Treatment Planning and Progress Reviews
- Complete individualized Treatment Plan Case Management goals and objectives for each client.
- Case Management goals and objectives on the Treatment Plan demonstrate that the plan for treatment was developed with the client.
- Treatment Plan problem areas, goals, and objectives address the client’s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment.
- Demonstrate use of natural supports in Treatment Plan.
Discharge Process
- Document termination of case management services.
- Update changed discharge criteria on the TPR.
- Discharge clients with no contact for 45 days.
Ongoing
- Coordination of appointments with psychiatrist and other primary therapists.
- Provide linking and coordination of community-based services.
- Transport clients as needed to community-based supportive, adjunctive services.
- Provide individual supportive, solution-focused interventions.
- Initiate contact to non-compliant clients within 24 hours of a missed appointment.
- Complete a progress note for all treatment services.
- Report medication issues to clinic psychiatrist and/or supervisor immediately.
- Consult with clinic psychiatrist within five days of the identification on a non-emergent medical condition.

Pay: $48,000.00 - $52,000.00 per year
