Job Overview
We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to serve as a Charge Nurse, leading nursing teams and ensuring the delivery of exceptional patient care across various healthcare settings. Responsibilities
- Supervise nursing staff during shifts, providing guidance, mentorship, and support to ensure high-quality patient care.
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews.
- Administer medications, including injections and following prescribed protocols while maintaining strict adherence to HIPAA and medical documentation standards.
- Coordinate care plans for patients with complex needs such as dementia, Alzheimer's disease, or those requiring memory or hospice care.
- Manage emergency situations by utilizing skills in airway management, acute pain management, triage procedures, and critical care protocols.
- Ensure compliance with long-term care regulations and documentation standards using EMR/EHR systems
- Collaborate with interdisciplinary teams to facilitate discharge planning, case management, diagnostic evaluations, and patient education.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing.
- Demonstrated expertise working with diverse populations including geriatric patients, individuals with developmental disabilities, or those requiring specialized dementia or memory care.
- Proficiency with EMR/EHR systems
- Strong knowledge of anatomy, physiology, infection control practices, and medication administration protocols.
- Ability to perform physical examinations, venipuncture (phlebotomy), sterile procedures, and catheterization confidently.
- Excellent communication skills for effective caregiver communication and patient assessment; ability to work well under pressure in fast-paced settings such as emergency room or urgent care environments.
