Join our dynamic healthcare team as a Registered Nurse or Licensed Practical Nurse specializing in Assisted Living Facility(ALF) and Memory Care (MC). Responsibilities
- Perform thorough patient assessments, including vital signs, physical examinations, and health histories to determine individualized care needs.
- Delegate care staff and med techs
- Administer medications and injections accurately following prescribed protocols.
- Manage acute pain and chronic conditions by implementing effective treatment plans aligned with best practices in geriatrics and memory care.
- Document all patient interactions meticulously within Electronic Health Record (EHR) systems such as Epic, Cerner, or Athenahealth, ensuring compliance with HIPAA and confidentiality policies.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and long-term care strategies for residents with complex needs like dementia or Alzheimer's disease.
- Provide specialized care for residents with developmental disabilities or behavioral health conditions, ensuring safety and dignity at all times.
- Support emergency response efforts by managing airway procedures, ventilator support, or urgent triage situations in accordance with Level I Trauma Center standards when applicable.
Qualifications
- Valid licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current state certification.
- Proven experience in long-term care facilities, skilled nursing facilities, or hospital settings such as ICU or emergency rooms.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
- Experience working with geriatric patients and individuals with dementia or memory impairments; Alzheimer’s and hospice care experience is highly desirable.
- Ability to perform venipuncture, phlebotomy, sterile procedures, and tube feeding; basic life support (BLS) certification required.
- Excellent patient care skills combined with critical thinking abilities for effective triage and urgent care situations.
