Job Summary
The Hospice CNA provides direct, hands-on personal care to hospice patients under the supervision of a Registered Nurse, in accordance with the patient's individualized plan of care. This role is essential to ensuring our patients remain comfortable, clean, and cared for with compassion during one of life's most vulnerable stages.
Schedule
We are hiring for both 12-hour shifts:
- Day Shift: 7:00 AM – 7:00 PM
- Night Shift: 7:00 PM – 7:00 AM
Shifts may include rotating weekends and holidays as part of our care schedule. Please indicate your shift preference when applying.
Key Responsibilities
- Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance in accordance with the patient's plan of care
- Take and record vital signs (temperature, pulse, respiration, blood pressure) and report changes in patient condition to the supervising RN
- Assist patients with feeding and monitor nutritional/fluid intake as directed
- Provide light housekeeping duties directly related to patient care (changing linens, tidying patient's immediate area)
- Observe and report changes in patient's physical or emotional condition to the RN Case Manager
- Provide emotional support and companionship to patients and families during end-of-life care
- Maintain accurate, timely documentation of care provided in accordance with agency policy and CMS Conditions of Participation for hospice care
- Follow the individualized plan of care as established by the interdisciplinary group (IDG)
- Maintain patient confidentiality in compliance with HIPAA
- Communicate professionally and compassionately with patients, families, and the care team
Qualifications
- Current, unencumbered Certified Nursing Assistant (CNA) certification in the State of Illinois
- Minimum of 1 year of CNA experience preferred; hospice or home health experience a plus
- CPR/BLS certification (or willingness to obtain)
- Valid driver's license, reliable transportation, and current auto insurance (for home-visit assignments)
- Ability to pass a background check, drug screening, and health screening (including required immunizations/TB testing) per Illinois Department of Public Health and CMS requirements
- Strong communication and interpersonal skills
- Compassionate, patient, and comfortable working with terminally ill patients and their families
- Ability to work independently and as part of an interdisciplinary care team
Physical Requirements
- Ability to lift, transfer, and reposition patients (up to [50] lbs, with assistance as needed)
- Frequent standing, bending, kneeling, and walking during shift
- Ability to work a full 12-hour shift, including overnight hours as scheduled
