Position Overview
As a Hospice Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.
What You Can Expect in This Role

Full-time Schedule: Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts

Patient Population: Adults - interest in training for an occasional pediatric patient

Manageable Caseloads: Daily visit requirements are below the state and national average

Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within Boise and it's surrounding regions

Additional Pay: On-call pay and applicable differential pay

Bonuses: Sign-on bonus and relocation assistance available (where applicable)

Qualifications

Education: ASN or BSN in Nursing

Experience: 2 years of RN experience, Hospice Case Management experience preferred

Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification

Other Requirements: Valid driver’s license and proof of auto insurance
