Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or resident’s personal assistive devices.
- Monitor and record vital signs, observe patient conditions, and report changes to healthcare professionals promptly.
- Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring proper food safety and hygiene standards are maintained.
- Administer medications accurately following care plans while adhering to HIPAA regulations to protect client confidentiality.
- Support clients with cognitive impairments by implementing memory care techniques for individuals with Alzheimer’s or dementia, including engaging activities and safety measures.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning living areas, laundry, and ensuring hygiene standards are met.
- Collaborate with healthcare teams to develop and follow personalized care plans that address each client’s unique needs and goals.
- Provide emotional support through active listening, companionship, and encouraging social interactions within the community setting.
Experience
- Prior experience in home health care, assisted living facilities, nursing homes, or group homes is highly desirable.
- Demonstrated knowledge of patient observation techniques, medication administration, and vital signs monitoring.
- Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, or memory-related conditions such as Alzheimer’s or dementia care.
- Hands-on experience using assistive devices like Hoyer lifts or resident lifts for safe patient transfers.
- Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information.
- Experience in providing long-term care or hospice support is a plus.
- Ability to perform heavy lifting when necessary to assist clients with mobility challenges safely.
