Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by observing vital signs, recording medical documentation accurately in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly.
- Administer medications following prescribed schedules and ensure proper medication management while maintaining compliance with HIPAA guidelines.
- Provide specialized care including Alzheimer's care, dementia care, hospice support, and end-of-life assistance to promote comfort and dignity.
- Support patients with medical procedures such as catheter care, wound care, and basic life support (BLS), including CPR when necessary.
- Perform light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment to uphold home & community care standards.
- Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met with empathy and professionalism.
Qualifications
- Proven experience working as a Home Health Aide or in assisted living facilities, nursing homes, or long-term care settings.
- Strong knowledge of medical terminology, patient observation techniques, and home health care regulations.
- Experience working with individuals with developmental disabilities or geriatrics is highly preferred.
- Certification in First Aid and CPR is required; additional training in hospice or dementia care is a plus.
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts.
- Familiarity with EMR/EHR systems for medical documentation and client communication.
- Excellent interpersonal skills with a compassionate approach to caregiving while maintaining patient confidentiality under HIPAA standards.
