Agency: Alternate Solutions Home Care – Fairfax
Schedule: Monday–Friday, 8:30 AM – 5:00 PM | On-call: Just 2 weekends per quarter
Territory: Northern Virginia (Ft. Belvoir, Lorton, Kingstown, Falls Church, Arlington, Alexandria City, Mt. Vernon, Alexandria)

How You'll Make an Impact
As a Physical Therapist (PT), you will deliver high-quality, one-on-one skilled care to patients in the comfort of their homes.
- Clinical Excellence: Conduct initial assessments (OASIS), build personalized plans of care, and deliver targeted treatments to restore function, relieve pain, and prevent hospital readmissions.
- Patient & Family Education: Empower patients and families through targeted therapy management, preventive care techniques, and proper equipment training (wheelchairs, walkers, orthotics).
- Team Collaboration & Leadership: Supervise PTAs and home health aides, coordinate with attending physicians, and participate in case conferences to maintain seamless continuity of care.
What We're Looking For
- Licensure: Active state license as a Physical Therapist (PT).
- Experience: Minimum of 1 year of PT experience in an acute care setting; home health experience is a plus.
- Skills: Strong clinical judgment, detail-oriented mindset, and effective communication skills.
- Requirements: Valid driver’s license, active auto insurance (listed driver), and reliable transportation for daily territory coverage.
