Patients First Home Care LLC is looking for Full-time and/or PRN Physical Therapists to provide in-home visits for the Port Charlotte and Punta Gorda areas.
Responsibilities
Provides a comprehensive patient examination/evaluation. Complete Oasis assessments timely and accurately. Reassess patient every 30 days and make necessary revisions to the plan of care in conjunction with the physician and documents the patient's continual needs and progress.
Provide patient oversight and supervision of Physical Therapy Assistants. Supervision of Home Health Aides with updates to care plan as needed.
Participate in patient care conference, updates and assists other members of the care team. Participate in services, chart audits, and quality improvement activities.
Requirements
- Current license to practice Physical Therapy in the State of Florida
- Graduated from an accredited physical therapy educational program
- One year of physical therapy experience. Preferably 1 year home health experience
- A level II background screening is required for this position https://info.flclearinghouse.com
- Current Driver's License with current auto insurance
- CEU on Elderly Abuse, Human Trafficking and Alzheimer's/Dementia
- TB test results
- current hands-on CPR certification. We do not accept any online course.

Pay: $80.00 - $90.00 per hour
