At NorthBay Health, we are seeking a Licensed Clinical Social Worker (LCSW) in Fairfield, CA to provide clinical social work services to hospital patients and their families. This role supports patients with complex psychosocial needs and collaborates with interdisciplinary teams to ensure effective care, improved outcomes, and safe transitions.
Responsibilities:
- Provide direct clinical social work services, including counseling, discharge planning, and case management
- Primarily support inpatient hospital units, including reviewing daily census to identify patients needing clinical intervention and education to improve outcomes and reduce readmissions
- Conduct comprehensive biopsychosocial assessments addressing emotional, psychological, social, financial, and environmental needs
- Develop, implement, and monitor individualized care and discharge plans based on patient strengths and needs
- Manage psychosocially complex patients across the continuum of care, identifying and addressing barriers to care and length of stay
- Collaborate with physicians, nurses, case managers, ED staff, and interdisciplinary teams, including triaging case requests at the VacaValley campus
- Initiate and discontinue 5150 involuntary psychiatric holds within NorthBay Medical Center, VacaValley Emergency Department, and inpatient units in accordance with Welfare and Institutions Code Section 5150
- Partner with Population Health to follow high-utilizer patients in the outpatient setting, ensuring team-based continuity of care
- Facilitate integration with community-based resources and collaborate with community partners to support optimal patient outcomes
- Communicate relevant patient information across the continuum of care to ensure continuity and appropriate follow-up

Compensation Structure
NorthBay Health uses a structured compensation framework. Staff-level positions use a step-based system (Steps 1–5) based on years of directly related experience, with Step 5 representing 20 or more years of experience in the role. Manager level and above positions are paid a fixed annual base salary and are eligible for a variable incentive compensation plan. Physician compensation is structured based on specialty and role requirements.
