Job Description
A1 Healthcare Hospice is seeking a compassionate and dependable Part-Time Medical Social Worker (MSW) with prior hospice experience to provide psychosocial support, counseling, and resource coordination to hospice patients and their families, primarily serving Sacramento and Placer Counties.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify the emotional, social, financial, environmental, and caregiver needs of hospice patients and their families.
- Provide ongoing emotional support, counseling, crisis intervention, and assistance with coping related to terminal illness, end-of-life concerns, grief, loss, and changes in condition.
- Assist patients and families with community resources, financial assistance programs, placement needs, transportation, advance care planning, and other applicable services.
- Provide education and guidance to patients and families regarding available healthcare, community, and elder-care resources.
- Provide patient-centered hospice care wherever the patient calls home, including private residences, assisted living facilities (ALFs), board-and-care homes, skilled nursing facilities (SNFs), and other residential settings.
- Collaborate closely with physicians, nurses, hospice aides, chaplains, and other members of the Interdisciplinary Team (IDT) to develop and maintain an individualized plan of care.
- Identify and communicate changes in psychosocial needs, safety concerns, caregiver needs, or other significant issues to the appropriate members of the hospice team.
- Assist patients and families with anticipatory grief and bereavement needs and coordinate additional support when appropriate.
- Advocate for patients and families while respecting their individual values, preferences, cultural needs, and goals of care.
- Participate in IDT meetings, care coordination, and case discussions as required.
- Complete all required clinical documentation accurately and within agency-required timeframes.
- Maintain patient confidentiality and comply with HIPAA, agency policies, and applicable hospice regulations.
- Maintain professionalism, compassion, and a patient- and family-centered approach at all times.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited program.
- Prior hospice experience is required.
- Knowledge of hospice philosophy, end-of-life care, grief, loss, and psychosocial support.
- Strong communication, counseling, assessment, and organizational skills.
- Ability to work independently in the field while collaborating effectively with the interdisciplinary team.
- Must have reliable transportation and be able to travel throughout the assigned service area.
Pay: $40.00 - $60.00 per hour
